Understanding Acne Scarring

Why scars form — and why they respond to treatment.

Acne scars form when inflammation damages the follicle wall and the surrounding dermis. Your body repairs the damage with collagen, but the repair is often uneven — producing depressions (atrophic scars), raised tissue (hypertrophic scars), or discolouration (post-inflammatory hyperpigmentation).

The treatable layer is the dermis — the structural foundation beneath the surface. Each of our three techniques reaches this layer differently, triggering collagen remodelling that gradually improves the appearance of scarring.

Ice pick scars

Deep, narrow channels that extend into the dermis. The most difficult type to treat — require precise, targeted technique and typically respond best to a combination approach over multiple sessions.

Boxcar scars

Wide depressions with sharply defined edges. One of the most common scar types. Respond well to RF Microneedling, which targets the structural layer where the depression originates.

Rolling scars

Wave-like undulation across the skin surface, caused by fibrous tissue pulling the dermis downward. Respond to both RF Microneedling and Sculptra — the latter addresses the volume deficit pulling the skin down.

Post-inflammatory hyperpigmentation (PIH)

Dark marks rather than textural scarring — a pigmentation response to inflammation, not a structural scar. Addressed with different modalities including BBL Heroic™ and VI Peels rather than microneedling alone. Your provider will assess at consultation.

Acne scarring is not one thing. Ice pick, boxcar, rolling, and hypertrophic scars each respond to different techniques — and many patients have more than one type. Your provider maps your scar pattern before recommending a single technique or combination. There is no one-size-fits-all protocol here.

Three techniques. Used alone or together.

RF Microneedling, Microneedling with PRF, and Microneedling with Sculptra are distinct approaches that can be combined across a treatment series. Patients with both surface texture and deeper volume loss often benefit from starting with RF Microneedling and layering in Sculptra once the surface has responded. Your provider builds the plan at consultation.

Your Treatment Journey

What to expect at every stage.

  • Stop blood thinners (aspirin, ibuprofen, fish oil) 2 weeks before treatment.
  • Stop topical exfoliants, retinol, and AHA/BHA 3–5 days before. Continue to avoid until skin has fully healed.
  • Avoid sun exposure. If you arrive tanned, your provider may require rescheduling.
  • If your skin tans easily, your provider may recommend pretreating with Brightening Pads for 2 weeks prior.
  • If you're being treated with PRF, hydrate well — aim for 2 litres of water per day for 24 hours prior.
  • Arrive with no jewelry, a low-cut top, and bare skin (SPF is okay).
  • Numbing cream is applied before treatment. Numbing time is typically 30–45 minutes.
  • If your treatment includes PRF, a small blood sample is drawn and processed in a centrifuge before needling begins.
  • The RF Microneedling or SkinPen device is applied across the treatment area. Some warmth or mild pressure is normal.
  • If Sculptra is part of your treatment, it is applied topically immediately after needling — not injected separately.
  • Total appointment time is 60–90 minutes depending on your selected technique.
  • 1–3 days: slight to moderate swelling and a sunburn-like reaction are common. Use an ice roller to minimise discomfort — starting the day after treatment, in the mornings.
  • Do not exfoliate or pick at texture. This may lead to scarring.
  • No makeup for 48 hours. No saunas, hot yoga, hot showers, or excessive sweating for 48 hours.
  • Keep skin covered from sun for 2 weeks. Physical SPF always.
  • 3–5 days: gentle cleanser, moisturiser, and physical SPF only. No active ingredients until skin has completely healed.
  • 5–7 days: no retinol, AHA/BHA, toning pads, or anything scrubby until fully healed.

Will my scarring come back?

Results can be long-lasting, but no treatment guarantees scars won't recur. New breakouts can cause new scarring. Maintaining a consistent skincare routine helps protect results. Individual results vary.

How many sessions will I need?

Can I treat active acne at the same time?

Active acne should be addressed first. Microneedling over active breakouts can spread bacteria and worsen scarring. Your provider will assess readiness at consultation.
